Position Overview
Are you a people leader who thrives on building strong teams and turning complex federal rules into workflows that actually work? Ceresti Health is hiring an empathetic, detail-oriented, and mission-driven Supervisor of GUIDE Coordination to lead our team of GUIDE Program Coordinators and own day-to-day compliance with the federal CMS GUIDE (Guiding an Improved Dementia Experience) Model.
In this remote role, you’ll lead the people who guide families through the multi-step onboarding journey after they’ve said “yes” to support — making sure every patient and caregiver moves through the process with clarity, care, and confidence. GUIDE is a rapidly evolving CMS model and Ceresti is a quickly scaling company, so we’re looking for someone who is agile, adaptive, and a natural problem solver — someone who can build the plane while flying it and bring the team along for the ride. This isn’t just a supervisory job — it’s a chance to shape how thousands of caregivers experience their first weeks in the GUIDE program.
Why This Role Matters
Caregivers of people living with dementia often feel overwhelmed and under-supported. Ceresti’s GUIDE program offers personalized coaching, care coordination, care planning, respite care, and resources at no cost to eligible families.
As Supervisor of GUIDE Coordination, you’re the operational heart of the GUIDE team, ensuring our coordinators have the structure, tools, and feedback they need to do their best work, and making sure Ceresti remains a trusted, fully-compliant GUIDE participant so families can keep accessing the care they deserve.
What You’ll Do
- Lead, manage, and hold accountable (LMA) the team of GUIDE Program Coordinators — hiring, onboarding, coaching, and performance management
- Set clear roles, productivity standards, and quality expectations; run consistent 1:1s, team huddles, and performance reviews
- Own day-to-day compliance with the CMS GUIDE Model: eligibility, alignment, and ongoing engagement
- Monitor and operationalize new and updated CMS GUIDE rules and guidance into clear workflows, policies, and training
- Maintain and administer the CMS GUIDE portal, ensuring accurate, timely beneficiary alignment and required reporting
- Track and report on GUIDE alignment and performance metrics, quality measures, and CMS benchmarks
- Collaborate with the Engagement team to establish KPIs that ensure compliance with GUIDE and payment benchmarks
- Collaborate with Revenue Cycle Management (RCM) to align documentation, coding, and billing with GUIDE payment requirements
- Audit charts for GUIDE compliance, with a sharp focus on required timelines and documentation completeness
- Oversee care plan development , ensuring plans are person- and caregiver-centered, clearly documented, and updated on schedule
- Track timeliness of dementia screenings, health assessments, neurology evaluations, tablet shipment, CMS submissions, and warm handoffs to Engagement Specialists and Coaches
- Provide structured feedback and coaching to coordinators based on audit findings, and close systemic gaps with targeted process and training improvements
- Partner with product management, analytics, engineering, and engagement partners to optimize platform workflows and reporting
- Identify compliance risks early and lead corrective action in partnership with leadership
- Anticipate, surface, and solve operational problems as the GUIDE Model evolves and Ceresti scales — translating ambiguity into clear next steps for the team
- Adapt to evolving processes and drive continuous improvement across the GUIDE program
The Right Fit
You’re a great match if you:
- Are agile, adaptive, and a natural problem solver — energized (not overwhelmed) by a rapidly evolving model and a quickly scaling company
- Stay calm in ambiguity and can build structure where none exists yet — turning new CMS guidance or shifting priorities into clear plans for the team
- Are an organized, accountable people leader who genuinely enjoys developing teams
- Read CMS guidance carefully and translate it into clean, repeatable workflows
- Use data and KPIs to spot problems early and drive improvement
- Communicate clearly and compassionately with staff, partners, and leadership
- Thrive in a dynamic, mission-driven, tech-enabled environment and adjust quickly to changes in workflows and priorities
- Enjoy working cross-functionally with clinical, engagement, RCM, and tech teams
- Understand (or are eager to deepen your understanding of) dementia care, caregiver needs, and CMS value-based programs
Qualifications
- Bachelor’s degree in nursing, social work, healthcare administration, public health, or a related field preferred
- 3-5+ years of experience in care coordination, case management, or population health, including work with CMS value-based or alternative payment models
- 2+ years of supervisory or team-lead experience, with a track record of coaching, developing, and holding staff accountable
- Strong working knowledge of CMS regulatory requirements, quality measures, and payment models; direct experience with the GUIDE Model strongly preferred
- Demonstrated ability to operationalize compliance by turning regulations into workflows, training, and SOPs
- Experience auditing clinical documentation and care plans for compliance and quality
- Comfortable working in a fast-paced, tech-enabled environment with EHR systems, CMS portals, and reporting/BI dashboards
- Demonstrates flexibility in response to changes in workflows, tools, and program requirements
- Familiarity with revenue cycle management and the link between documentation, coding, and reimbursement is a plus
Working Conditions
- Remote work environment with flexible scheduling options across time zones
- Standard business hours with potential for some evening or weekend availability around CMS deadlines or audits
- Requires dedicated, private workspace suitable for confidential team and family discussions
- Occasional travel may be required for team meetings, training, or audit activities
